Job summary

Neighborhood Health Center is seeking an LPN to provide nursing support under supervision at multiple Western New York sites. You will perform health teaching, counseling, and restorative care while collaborating with a licensed provider.

You will be based in Blasdell and Hamburg with travel to other Neighborhood sites as needed. The role emphasizes teamwork, caring for patients with respect and compassion.

Qualifications

• Graduated from an accredited nursing program (1–2 years).
• Current NYS License as a Practical Nurse (LPN).
• Current CPR certification required.

Responsibilities

• Perform nursing tasks under direction of a registered nurse or licensed provider.
• Administer medications and provide prescribed treatments.
• Perform immunizations and injections.
• Assist in facilitating patient flow for providers.
• Maintain care and use of supplies and equipment.
• Place and document PPD and CLIA-waived tests.
• Comply patient information in absence of an RN and consult with provider.

About the Role:

As an LPN at Neighborhood, you’ll use your training, organizational skills, attention to detail, and influencing skills as you perform nursing tasks and help ensure an excellent patient experience. Responsibilities include:

• Nursing tasks within the framework of case finding, health teaching, health counseling and provision of supportive and restorative care under direction of a registered professional nurse or licensed provider
• Administer medications and provide treatments prescribed by providers
• Perform immunizations and injections
• Assist in facilitating patient flow for providers
• Maintain the care and use of supplies, equipment, etc.
• Perform all CLIA waiver tests and ensures all tests are up to date
• Place and document PPD
• Comply patient information in the absence of an RN and consult with medical provider

You will be primarily based at our Blasdell and Hamburg locations and you will travel to other Neighborhood sites in Western New York as needed.

Education and Skills to be an LPN:

• Graduated from an accredited one or two year nursing school
• Current NYS License as a Practical Nurse
• Current CPR certification

What We Offer:

Compensation: $26.75 an hour based on a full-time, 40 hour work week

Plus, this position offers a sign-on bonus for external hires, paid in installments throughout the employee's successful first year of employment.
