Job summary

The Indigo at Elmhurst is seeking a Licensed Practical Nurse (LPN) to provide exceptional care to residents. The role requires administering medications, monitoring vital signs, and assisting with personal care activities.

Qualifications

Licensed Practical Nurse

We are seeking a Licensed Practical Nurse. Memory Care experience is strongly preferred.

Key Responsibilities

• Administer medications and treatments; document accurately in EHR.
• Take vital signs and monitor resident condition.
• Assist with resident assessments, nursing calls, and incident reporting.
• Provide personal care: bathing, dressing, feeding, walking, transferring, and other daily living activities.
• Collect lab samples and follow up on results with residents, families, and physicians.
• Prepare and check food trays per prescribed diets.
• Maintain cleanliness and safety in resident rooms; respond to resident calls.
• Support and train staff as directed by HCC.
• Comply with HIPAA, company policies, and federal/state regulations.
