Job summary

Elizabeth Park Rehabilitation and Nursing LLC is seeking a compassionate Licensed Practical Nurse (LPN) for a 7:00 AM to 3:30 PM shift. You will provide direct patient care, administer medications, and support rehabilitation and long-term care goals in a collaborative team environment.

The position requires NJ LPN licensure, a high school diploma or equivalent, and 1+ year of LTC or rehab experience. Strong communication and physical stamina are essential to succeed in this role.

Qualifications

• Current LPN licensure in New Jersey.
• High school diploma or equivalent; ADN/BSN degree preferred.
• 1+ year of experience as an LPN in a long-term care or rehabilitation setting.
• Strong communication and interpersonal skills.
• Ability to lift, transfer, and reposition patients.

Responsibilities

• Provide direct care to patients and residents, including administering medications, wound care, and other treatments as prescribed by physicians.
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
• Monitor and report patient progress, identifying areas for improvement and making recommendations for modifications to care plans.
• Collaborate with other healthcare professionals, including RNs, PTs, OTs, and social workers, to ensure comprehensive care.
• Participate in patient and family education, promoting effective communication and empowerment.
• Maintain accurate and detailed records, adhering to regulatory and facility standards.
