Job summary

Liberty Resources' Integrated Health Center in Syracuse, NY seeks a Licensed Practical Nurse (LPN) to support a busy primary care team. The LPN will assist providers, perform intake, screenings, administer medications, and support care coordination within an EMR-enabled clinic.

Requirements include NY LPN license, 1+ year of clinical nursing experience (preferably in primary care), and BLS. Competitive pay and a comprehensive benefits package are offered.

Qualifications

  • Graduate of an accredited LPN program.
  • Current New York State LPN license in good standing.
  • Minimum of one year of clinical nursing experience preferred, especially in primary care or ambulatory settings.
  • Eligible for full and unconditional participation in Medicare and Medicaid programs.
  • Strong assessment, organizational and patient education skills.
  • Excellent verbal and written communication; ability to work in a multidisciplinary team.
  • Proficiency with EMR systems and related health IT.
  • Current BLS certification is required.
  • Valid NYS driver's license and reliable transportation when needed.

Responsibilities

  • Assist providers with patient examinations, procedures, and treatments.
  • Perform patient intake: vitals, histories, meds reconciliation, and chart patient concerns.
  • Provide health screenings and preventive care within scope of practice.
  • Administer meds, immunizations, and treatments as ordered.
  • Perform phlebotomy and specimen collection as trained.
  • Educate patients on preventive health, disease management and healthy lifestyles.
  • Coordinate care by communicating with patients, families, pharmacies, and specialists.
  • Assist with medication refills, prior authorizations, and referrals to specialists.
  • Review labs and imaging with providers and support follow-up care.
  • Monitor patients with chronic conditions and schedule follow-ups and screenings.
  • Maintain EMR documentation accurately and timely.
  • Ensure compliance with quality measures, infection control, and policies.
  • Respond to urgent situations and assist with emergencies as needed.
  • Participate in meetings and quality improvement activities.
  • Perform other duties as assigned.

Licensed Practical Nurse (LPN) Position Summary:

The Licensed Practical Nurse (LPN) functions as an integral member of the Primary Care Team, providing clinical support and direct patient care under the supervision of a registered nurse and/or primary care provider. The LPN assists in delivering comprehensive healthcare services, promotes preventive care and wellness, supports chronic disease management, and helps ensure efficient clinic operations.

License Practical Nurse (LPN) Job Responsibilities:
  • Assist providers with patient examinations, procedures, and treatments.
  • Perform patient intake, including obtaining vital signs, medical histories, medication reconciliation, and documenting patient concerns.
  • Provide health screenings and routine clinical assessments within scope of practice.
  • Administer medications, immunizations, and treatments as ordered by the provider and in accordance with agency policies.
  • Perform phlebotomy and specimen collection procedures for which the LPN is trained.
  • Provide patient education regarding preventive health, chronic disease management, medications, treatment plans, and healthy lifestyle choices.
  • Support care coordination by communicating with patients, families, pharmacies, specialists, and community resources as appropriate.
  • Assist with medication refills, prior authorizations, and referrals to specialty care providers.
  • Review laboratory and diagnostic results with providers and assist with patient follow-up as directed.
  • Monitor patients with chronic health conditions and facilitate follow-up care and preventive screenings.
  • Maintain accurate and timely documentation within the electronic medical record (EMR).
  • Ensure compliance with clinical quality measures, infection control standards, and agency policies.
  • Respond appropriately to urgent medical situations and assist with emergency interventions as needed.
  • Participate in team meetings, quality improvement initiatives, and other clinic activities.
  • Perform other duties, responsibilities, and related tasks as assigned.
Licensed Practical Nurse (LPN) Qualifications:

Salary: starting at $60,000
