Job summary

ERC Pathlight in Illinois is seeking a part-time Licensed Practical Nurse for night shift support across locations. The LPN will provide direct patient care under supervision, perform vital signs, administer medications, and assist with environment safety and crisis management in alignment with standards of care.

Responsibilities include collaborating with the treatment team to support patient-centered plans, float across ERC and Pathlight, and participate in on-call shifts as needed.

Qualifications

• High school diploma or GED (Required)
• Active LPN license in the state job is held (Required)
• BLS certification (Required)
• Ability to keep patient information confidential (Required)
• Ability to remain calm in stressful situations (Required)

Responsibilities

• Performs nursing care within scope of practice to assigned patients under the supervision of an RN, APP, or provider
• Manages patient assignment within assigned shift
• Performs and documents vital signs, responses to treatment, education and nursing care and assists with other basic medical care tasks
• Reports abnormal changes in patients or patients’ health and mental status to RN or provider in a timely manner
• Receives, places and documents provider orders/communication in a timely manner using approved abbreviations
• Administers medications, injections, places and maintains nasogastric tube feeds, and provides wound care according to facility and state regulations
• Contributes to individualized treatment plans with the ERC multidisciplinary team
• Responds to emergency situations until emergency response team arrives on scene

Whatyou’llbe doing:

*Part time, Night Shift

The Licensed Practical Nurse will provide andassistin direct patient care under the supervision of a Registered Nurse,APPor physician. The LPN will provide medical management of patients andassistwith environmental safety and crisis management for the patient populationin accordance withthe legal scope of practice and within established standards of care,policiesand procedures. The LPN willdemonstrateperformance consistent with the Mission,Visionand Strategic Plan of the organization,demonstratequality and effectiveness in work habits and clinicalpracticeand treat coworkers, patients,familiesand all members of the health care team with dignity and respect. The LPN will partner with the treatment team to provide a consistently therapeutic milieu.

The LPN will crosstrainacross ERC and Pathlight and may berequiredto float or be assigned to other locations/facilities within the Region. The LPN maybe requiredto work on-call shifts based on business needs,in accordance withapplicable federal, state, and local labor laws.

Essential duties and responsibilities:

• Performs all nursing care within scope of practice to assigned patients under the supervision of an RN, APP, or provider

• Manages patient assignment within assigned shift

• Performs and documents vital signs, responses to treatment, education and nursing care andassistswith other basic medical care tasks assigned

• Reports abnormal changes in patients or patients’ health and mental status to RN or providerin a timely manner

• Receives, places and documents provider orders/communication intimelymanner using approved abbreviations

• Effectively labels,collectsand processes patient specimens

• Administers medications, injections, places andmaintainsnasogastric tube feeds, and provides wound care according to facility and state regulations

• Mayassistlicensed provider with patient examinations and treatments by positioning, restraining patients, andassistingas directed

• Performs all aspects of patient care in an environment thatoptimizespatient safety and reduces the likelihood of medical/health care errors

• Ensures that care reflects goals of patient outcome activities

• Contributes to individualized treatment plans in conjunction with the ERC multidisciplinary team

• Participates in daily controlled substance count

• Aidesin transition/transfer of patients to and from other units or facilities

• Collaborateswith staff toassistwith transportation arrangements for patient appointments outside of ERC

• Responds to emergency situations until emergency response team (911) arrives onscene

Musthaves:

• High school diploma or GED (Required)

• Active LPN license in the state job is held (Required)

Bonusqualities:

• 1+ years of experience working with in the mental health field, IV skills/certification, knowledge/experience with nasogastric tubes (Preferred)

The compensation range for this position is based upon candidate experience and market expectations.

Illinois Pay Range

$29.50—$36 USD
