Job summary

Claremont Nursing & Rehabilitation Center is seeking a Weekend LPN to provide compassionate, skilled nursing care to residents on weekends. You will work under the supervision of our nursing staff, delivering medications, monitoring residents, and supporting daily activities in a subacute and long-term care setting.

The role requires an active PA LPN license, weekend availability, and strong communication with residents and families.

Qualifications

• Current LPN license in Pennsylvania.
• High school diploma or equivalent.
• Previous experience in a long-term care or rehabilitation setting preferred.
• Ability to work weekends and flexible scheduling.
• Excellent communication and interpersonal skills.
• Physical ability to lift, transfer, and move patients as needed.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring.
• Assist with daily living activities, such as bathing, dressing, and grooming.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Maintain accurate and up-to-date patient records.
• Provide emotional support and companionship to residents and their families.
• Participate in quality improvement initiatives to ensure optimal patient outcomes.

About the Role

As a Weekend LPN, you will be responsible for providing high-quality patient care to our residents under the supervision of our nurses.
