Job summary

Berlin Rehabilitation and Healthcare Center is hiring a Licensed Practical Nurse (LPN) for full‑ and part‑time shifts in a 128‑bed sub‑acute and long‑term care setting.

We support our nurses with competitive pay: $30.00–$35.00 per hour, same‑day pay options, and shift differentials. BLS CPR and NJ LPN licensure are required, with prior long‑term care experience preferred.

Qualifications

• Active LPN license in New Jersey.
• BLS/CPR certification required.
• Experience in long‑term care or subacute settings preferred.
• Familiarity with PCC platform.
• Able to manage multiple patients in a fast‑paced facility.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow individualized care plans and work with interdisciplinary team.
• Administer medications per policy and treatment plan.
• Monitor residents and report changes promptly.
• Maintain accurate documentation and regulatory compliance.
• Support a safe, respectful environment for residents and families.

PER-DIEMS: 7 AM - 3 PM, 3 PM - 11 PM, 7 AM - 7 PM, 7 PM - 7 AM

$30.00 - $35.00 per hour (all inclusive)

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of New Jersey.

• Valid BLS Certification (CPR) required.

• Prior experience in Long-Term Care, Sub acute setting.

• Prior experience with PointClickCare (PCC).

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
