Job summary

Live Well Homecare is seeking a Licensed Practical Nurse (LPN) to support patients and assist medical staff in a clinic setting. Responsibilities include patient prep, injections, medication administration, and maintaining accurate records.

The ideal candidate will have completed an accredited LPN program and hold an active LPN license, with preferred hospital/clinical experience. Examinations, triage, referrals, and inventory management are part of the daily workflow to ensure smooth operations

Qualifications

• Graduate of an accredited practical nursing program.
• Licensed as an LPN in accordance with state regulations.
• Preferred experience in hospital or clinical settings.

Responsibilities

• Greet patients, prepare for examinations, record health histories, and note abnormalities.
• Assist with supplies, equipment, and procedures, including administering injections and medications.
• Observe and report patient conditions and reactions to treatments.
• Maintain accurate patient records, schedule tests, coordinate referrals, and obtain pre-authorizations.
• Collect lab specimens, deliver results, and communicate with patients regarding test outcomes.
• Restock and clean exam rooms, follow infection control policies, and manage inventory of medications and supplies.
• Support provider during physical exams, instruct patients and families, and handle phone triage and referrals.
