Job summary

Grace Lutheran Communities in Altoona, WI, is hiring a Licensed Practical Nurse (LPN) to join the River Pines team on part-time PM/NOC shifts. Starting wages are $28.00–$33.50 per hour, based on experience, with benefits and wage options.

This role focuses on compassionate, resident-centered care and collaboration with the nursing team. The LPN will perform basic nursing tasks, document care activities, and support care planning while ensuring high-quality care for residents.

Qualifications

• Licensed Practical Nurse (LPN) license required.
• Ability to perform and document basic nursing skills per care plans.
• Provide direct resident care and collaborate with RN on changes in condition.

ESSENTIAL FUNCTIONS:

• Perform and document all basic technical and emergency nursing skills within the framework of licensure and as outlined on individual care plans
• Document care delivery observations, medications, and treatments promptly and accurately
• Provide direct resident care as needed
• Request RN assessment/consultation for resident who show a change of condition
• Use equipment and supplies in a precise, accurate and calm manner
• Assume charge nurse duties when assigned including, but not limited to, adjusting assignment and schedule to best meet needs of residents and appropriate use of personnel
• Under direction of the RN, monitor an document health status of all residents who are unstable or requiring intervention
• Participate in care planning
• Obtain needed intervention and monitor process or order follow through
• Replace absent staff when necessary
• Has access to confidential information/records and maintain confidentiality
• Project a positive image of the organization in dealing with residents/clients, co-workers, and public
• Other duties as assigned
