Job summary

Ambercare Homecare in Albuquerque, NM is seeking a Licensed Practical Nurse to join our growing team. You will provide direct patient care and implement care plans under supervision, documenting per plan of care.

Requirements include graduation from an accredited LPN program, NM licensure, and CPR certification. A minimum of three years of nursing experience (home health or medical/surgical preferred) is desired. Valid driver's license and reliable transportation are required.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience; home health or medical/surgical preferred.
• Currently licensed as an LPN in NM.
• Possess and maintain valid CPR certification.
• Must have reliable transportation, valid driver's license and appropriate automobile insurance.

Responsibilities

• Provides direct patient care as defined by state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Documents accurately and timely per the plan of care.
• Educates patient and family on self-care per diagnosis and plan of care.
• Coordinates home health services and communicates needs to RN/Clinical Supervisor.
• Uses equipment with aseptic technique and proper care.
• Participates in personal and professional growth.
• Performs other duties as assigned by the RN.

What You’ll Do:

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
