Job description

Licensed Practical Nurse

Location: Adel Acres – Adel, IA

Position Purpose

The LPN is responsible for providing residents with the highest quality of direct routine care, under the supervision of the RN, and in accordance with BON standards, the Residents’ Bill of Rights, and the facility Quality Assurance and Performance Improvement (QAPI) program. The LPN participates in the development and modification of the resident Plan of Care, implements interventions delegated, ordered, or prescribed by the RN or physician, and collects resident health data to report changes and responses to the RN.

Essential Functions
Plans of Care
  • Conduct focused assessments of residents through data collection and comparison from interviews, observation, and medical records.
  • Participate in drafting the integrated, resident-centered Plan of Care.
  • Facilitate the Plan of Care through completion of nursing responsibilities, clearly communicating resident needs to primary caregivers and reporting changes to the RN.
  • Maximize quality of living through continual monitoring and documentation of resident care and correlated recommendations.
Ensuring Adequate and Proper Nursing Care
  • Ensure integration and effectiveness of the resident’s Plan of Care by implementing and evaluating nursing interventions with the interdisciplinary team.
  • Provide the highest quality care to all residents, regardless of race, gender, sexual orientation, religion, age or culture.
  • Offer health promotion and disease prevention information to residents.
  • Report suspicions of abuse, neglect, or misappropriation as per the Residents’ Bill of Rights.
  • Maintain awareness of facility QAPI goals and promote the program.
  • Promote a resident-centered experience and protect residents’ dignity, privacy, and autonomous decision-making.
  • Interact regularly with residents’ families or support networks to report changes in health or care.
  • Guide nursing staff on proper body mechanics, safe lifting techniques, and use of equipment.
  • Follow OSHA guidelines and infection control policies to maintain a safe environment.
  • Ensure physician orders are strictly followed and that records are complete, secure, and compliant with HIPAA.
  • Document nursing orders, medication administration, and incident reports accurately and promptly.
Supervision and Management
  • Evaluate supportive nursing staff practices, articulate expectations, and provide corrective guidance.
  • Prioritize work assignments to match staff availability and resident acuity.
  • Ensure proper staffing levels and assist as needed with direct patient care.
  • Follow up on delegated tasks and review departing staff reports.
  • Provide nursing reports on daily resident care needs and significant changes in condition.
  • Implement disaster procedures when required.
  • Report incidents and errors involving residents, staff, visitors, or self promptly.

Qualifications
  • Completion of a professional nursing education program approved by the Board of Nursing for the state in which the facility is located.
  • Current License as a Practical Nurse for that state.
  • Current certification in Cardiopulmonary Resuscitation (CPR) and emergency care.
  • Preferred: Experience in skilled nursing and/or assisted living community.
  • Leadership: ability to achieve goals through staff motivation.
  • Resilience: openness to feedback and ability to course‑correct.
  • Personability: ability to relate to residents, staff, and families.
  • High emotional intelligence (EQ) and active listening skills.
  • Proficient reading, writing, speaking, and understanding of English.
