Job summary

EmpowerMe Wellness seeks licensed therapists to deliver therapy and wellness services to older adults within our EmpowerMe 360 model. You’ll conduct assessments, build personalized care plans, and collaborate with a multidisciplinary team to support residents’ independence and safety in their living environments.

Responsibilities include care coordination, telehealth support, and maintaining high standards of professional care.

Qualifications

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Strong clinical skills with a collaborative spirit.
