Highland Care Center is seeking a Licensed Physical Therapist to provide mobile outpatient PT services in patients' homes and communities in Utah. You will assess, diagnose, and treat a range of impairments using evidence-based techniques to help patients regain mobility and independence.

The role requires a Utah PT license, at least 1 year of outpatient experience (preferably mobile), and a graduate PT degree. Certification in relevant PT areas is welcomed as part of professional growth.

Qualifications

• Licensed Physical Therapist (PT) with a valid license to practice in the state of Utah.
• Minimum 1 year of experience in outpatient physical therapy, preferably in a mobile setting.
• Graduate degree in Physical Therapy from an accredited program.
• Certification in relevant areas of physical therapy.

Responsabilités

• Conduct thorough assessments and evaluations of patients to identify physical therapy needs and develop personalized treatment plans.
• Provide skilled physical therapy services, including exercises, modalities, and manual therapy techniques.
• Collaborate with patients, families, caregivers, and healthcare teams to ensure comprehensive care and effective communication.
• Monitor patient progress and adjust treatment plans as necessary to optimize outcomes.
• Maintain accurate records and documentation of patient care and services provided.
• Stay up-to-date with industry developments, research, and best practices to continually improve skills and knowledge.
