Job summary

Life Care Centers of America in Coos Bay, OR is seeking a licensed Physical Therapist to join our collaborative post-acute care team. You will implement treatment plans, document progress, and work with a multidisciplinary staff to help patients regain function and independence.

New grads welcome; CPR certification required or obtained on orientation. Prior experience in a rehab setting is preferred; strong communication and patient-centered care are essential.

Qualifications

  • Graduate of an accredited program in physical therapy (ASPT).
  • Active state licensure in good standing; licensure must be maintained throughout employment.
  • One year of experience preferred; new grads welcome.
  • CPR certification on hire or during orientation; must remain current during employment.

Responsibilities

  • Follow PT treatment plans under supervision of the supervising PT.
  • Chart patient progress timely and accurately.
  • Utilize therapy software appropriately.
  • Provide excellent customer service to patients.
  • Demonstrate good body mechanics and safety.
  • Collaborate with interdisciplinary team; coordinate with staff and families.
  • Read, write, and speak English.

Setting & Population Served
  • Long term care for residents who require ongoing skilled support and compassionate daily assistance
  • Short term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury
  • Collaborative care through an interdisciplinary team approach with nursing, therapy, and medical staff working together
  • A diverse patient population including individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs

Patient mix varies slightly by facility. As a therapy professional, you will help patients achieve functional progress and improve quality of life within a supportive, patient-centered environment.

Education, Experience, And Licensure Requirements
  • Graduate of an accredited program in physical therapy (ASPT)
  • Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
  • One (1) year experience preferred; new grads welcome to apply
  • CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
Specific Job Requirements
  • Participates in community health matters/projects as appropriate
  • Must be proficient in Microsoft Word, Excel, and e mail
  • Liaisons with patients, families, support departments, etc., to adequately plan for patient needs
  • Demonstrate good body mechanics at all times
  • Make independent decisions when circumstances warrant such action
  • Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
  • Implement and interpret the programs, goals, objectives, policies, and procedures of the department
  • Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
  • Maintains professional working relationships with all associates, vendors, etc.
  • Maintains confidentiality of all proprietary and/or confidential information
  • Understand and follow company policies including harassment and compliance procedures
  • Displays integrity and professionalism by adhering to Life Care’s Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training

  • Assist in the evacuation of patients

  • Concentrate and use reasoning skills and good judgmentCommunicate and function productively on an interdisciplinary team
  • Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
  • Read, write, speak, and understand the English language
