Job description

Physical Therapist
Full-Time | MonteCedro | Altadena, California
Great Physical Therapy Helps People Keep Living the Life They Love.

Prava is looking for a Full-Time Physical Therapist to join our team at MonteCedro, an exceptional Life Plan Community in Altadena, California.

MonteCedro is designed for people who aren't ready to slow down.

It's a vibrant community built around wellness, connection, creativity, lifelong learning, and independence—with opportunities for fitness, swimming, walking, dining, the arts, social connection, and continued engagement.

That's exactly where we believe physical therapy belongs.

Not on the sidelines.
In the middle of life.

Treat the Person. Not Just the Diagnosis.

Your residents are already doing amazing things.

They're taking fitness classes.
Walking the campus.
Swimming.
Going to performances.
Meeting friends for dinner.
Taking classes.
Gardening.
Traveling.
Spending time with family.

And when an injury, illness, balance problem, weakness, or change in mobility gets in the way—

you help them get back to living.

That's the opportunity.

Your Role

As a Prava Physical Therapist, you'll provide exceptional clinical care while becoming a trusted part of the MonteCedro community.

You'll have the opportunity to:

  • Complete comprehensive physical therapy evaluations
  • Develop individualized plans of care based on clinical findings and resident goals
  • Provide skilled treatment for balance, gait, strength, mobility, endurance, and functional performance
  • Help residents recover following illness, injury, hospitalization, or a change in functional status
  • Build treatment around meaningful, real-world goals
  • Provide resident and caregiver education
  • Collaborate with nursing, wellness professionals, care teams, families, and other clinicians
  • Identify changes in mobility and function that may benefit from skilled intervention
  • Support fall prevention, mobility, wellness, and independence initiatives
  • Incorporate residents' actual environments and daily routines into treatment
  • Communicate clearly with residents, families, and the interdisciplinary team
  • Document efficiently using Prava's modern technology and AI-enabled tools
  • Contribute to a clinical culture built around exceptional care and continuous learning
Think Beyond the Diagnosis.

A resident may come to you because they're weak.

But weakness isn't the goal.

Maybe they want to walk to dinner.

Maybe they want to return to the pool.

Maybe they want to take their morning walk again.

Maybe they’re nervous about falling.

Maybe they want to travel with their spouse.

Maybe they simply want to feel like themselves again.

That's what you're treating.

The impairment is only part of the story.

Be the Therapist Who Notices.

Notice the resident who stopped attending exercise class.

The person who isn't walking as far as they used to.

The resident who has become less confident navigating the community.

The family member who says:

"She's just not moving like herself anymore."

An embedded Prava therapist doesn't wait for everything to become a crisis.

You know the residents.
You know the community.
You notice change.
And when therapy can make a difference, you act.

We're Looking for a PT Who:
  • Holds a current California Physical Therapist license
  • Has strong clinical judgment
  • Loves working with older adults
  • Thinks functionally rather than simply diagnostically
  • Builds genuine relationships with residents
  • Communicates exceptionally well
  • Is comfortable working independently
  • Collaborates naturally with interdisciplinary teams
  • Is curious and committed to continued clinical growth
  • Believes residents should be challenged appropriately
  • Understands that independence, dignity, and choice are fundamental to good care

Senior living experience is a plus—but it isn't everything.

If you're an excellent PT who wants to practice differently, we want to meet you.

Why Prava?
Because Great Clinicians Deserve Great Infrastructure.
Technology That Works for You

Prava combines EMR, clinical workflows, and AI-enabled documentation designed to reduce administrative burden and give you more time to focus on residents.

Clinical Support

You'll have access to clinical standards, resources, mentorship, and a growing community of rehabilitation professionals.

Operational Support

Prava supports the infrastructure behind the practice so you can focus your energy where it belongs:

With residents.

An Embedded Model

You're not a therapist who drives in, treats a schedule, and disappears.

You'll become a familiar face at MonteCedro.

A clinical resource.
A trusted partner.
Part of the community.

Room to Grow

As Prava grows, so can your career—with opportunities for mentorship, clinical leadership, education, community development, and more.

Don't Count Visits. Count Impact.

We don't want you measuring your day only by how many visits you completed.

We want you thinking about:

Who is moving better because I'm here?

Who feels more confident?

Who got back to something they love?

Who gets to stay independent a little longer?

Who is participating in life because therapy made it possible?

That's the work.

And MonteCedro is a pretty extraordinary place to do it.
