Qualifications

Responsibilities

• Evaluate medication orders for appropriateness and safety.
• Ensure medications are properly dispensed, labeled and stored.
• Recommend therapy changes to physicians when appropriate.
• Maintain patient databases with medical history and labs.
• Investigate and resolve medication-related queries.
• Communicate with nursing staff, physicians and other health professionals.
• Supervise medicine preparation and controlled substances register.
• Ensure security and handling of medications.
• Perform on-call duties as required.

Responsibilities/Duties:

• Evaluate medication orders for appropriateness of drug, dosage, potential interactions and proper administration
• Maintain dispense medications are correctly supplied and labeled
• Make recommendations for therapy changes to the physician and client facilities staff, if necessary
• Maintain customers’ databases including but not limited to medical history, clinical, laboratory data, etc
• Investigate and resolve medication-related queries
• Communicate with facilities’ nursing staff, physicians and other healthcare team members regarding medication-related queries
• Supervise the preparation of any medicines
• Maintain a register of controlled drugs for legal and stock control purposes
• Maintain proper security conditions for medications
• Perform “on-call” duties when required

Requirements:

• Licensed Pharmacist in the State of Texas
• Long Term Care experience preferred
• Previous experience in Hospital and Retail pharmacy or other medical fields
• Strong analytical and critical thinking skills
• Excellent written and verbal communication skills
• Strong attention to detail
• Strong organizational skills
• Computer skills
• Framework / ECM software knowledge a plus
• Creating a safe, effective environment
