Qualifications

• Valid OT license in the state of practice.
• Graduate of ACOTE accredited program; NBCOT eligible or certified.

Responsibilities

• Complete evaluations, assessments, and treatments per the OT care plan.
• Complete documentation accurately and timely.
• Follow policies related to scope of practice.
• Recognize priorities in patient care and act with initiative.
• Complete orientations, in-services, and trainings as directed.
• Perform other duties as directed by supervisor.
