Job summary

EmpowerMe Wellness is seeking licensed therapy professionals to join our EmpowerMe 360 team, delivering integrated therapy and wellness services to older adults in senior living communities.

You’ll conduct assessments, design personalized care plans, and collaborate with medical and wellness providers to help residents live stronger, safer, and more independent lives. Flexible schedules and opportunities for leadership and professional development are available.

Qualifications

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Ability to conduct assessments and create personalized care plans.
• Collaborative team player with strong communication and interpersonal skills.

• Support care coordination with wellness check-ins, caregiver communication, and telehealth support.

• Contribute to program development and continuous quality improvement.
• Help residents maximize independence, confidence, and satisfaction.
