Renewal Rehab is seeking a Licensed Occupational Therapist to join our collaborative team serving primarily seniors in skilled nursing and post-acute care settings. In this patient-centered role, you will evaluate and treat residents, develop individualized, outcome-focused plans of care, and help patients maximize safety, independence, and quality of life.

You will collaborate closely with an interdisciplinary team, document in EMR, and educate patients and families.

Qualificações

• Illinois-licensed occupational therapist.
• Experience serving seniors in skilled nursing or post-acute settings preferred.
• Strong communication and patient education abilities.

Responsabilidades

• Evaluate and treat residents to improve safety, independence and quality of life.
• Develop individualized, outcome-focused plans of care.
• Collaborate with an interdisciplinary team and document in the EMR.
