Job summary

South Pacific Rehab Services is seeking a licensed Occupational Therapist to join our PRN team in a SNF/Long-Term Care setting. You will evaluate patients, create personalized care plans, and collaborate with colleagues to achieve optimal outcomes.

Qualifications

• Must hold an OT license in the state of practice.
• NBCOT certification is required where state mandates.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are obtained and up to date.

What You'll Do:

Qualifications:

• Degree in Occupational Therapy (BS, MS, MA, or OTD) from an AOTA-accredited program
• Certified by the NBCOT as an OTR, if required by the state
• Current license as an Occupational Therapist in the state of practice
• Strong communication and organizational skills
