Job summary

Qualifications

• Master's Degree in Social Work (MSW) from a CSWE-accredited program required.
• 2-4 years of clinical social work experience in a healthcare setting preferred.

Responsibilities

• Conducts psychosocial assessments across age groups to evaluate emotional, social, and financial needs.
• Provides counseling, support, and resource education to patients and families.
• Participates in interdisciplinary discharge planning, coordinating post-acute care services.
• Facilitates referrals to internal and external services including mental health and community resources.
• Manages high-risk referrals and complex psychosocial cases.
• Provides crisis intervention and emotional support for acute distress or trauma.
• Collaborates with physicians, nurses, and case managers to support care planning and transitions.
• Participates in quality improvement and psychosocial outcomes tracking.
• Maintains accurate documentation in the medical record.
