Job summary

Qualifications

• Graduate of an accredited Master of Social Work program.
• Current LMSW license in Texas required.
• ACMA certification from ACMA preferred.
• Experience in hospital social work and discharge planning.

Responsibilities

• Assess psychosocial risk factors of patients and families.
• Develop and carry out multidisciplinary discharge plans.
• Provide crisis intervention and mental health assessment.
• Coordinate with physicians, case managers and nurses.
• Advocate for patient empowerment and community resource linkage.
• Assist with transition planning for post-acute care.
