Job summary

Ford's Hometown Services in Clifton, NJ is seeking LCSW/LSW and/or LPC/LACs for a community-based outpatient clinic. Shifts available Monday–Saturday with both in-office and at-home telehealth options.

Join a friendly, supportive team and engage in comprehensive intake assessments, therapy, interdisciplinary collaboration, treatment planning, and timely documentation. Supervision for licensure is provided.

Responsibilities

• Perform comprehensive intake assessments.
• Provide therapy.
• Collaborate with inter-disciplinary team.
• Partner with clients in meaningful treatment planning.
• Collect and communicate clinical information necessary to secure authorization for services with funding entities.
• Perform all written documentation including assessments, progress notes, treatment plans, status reports, etc. as required and within expected timeframes.
• Secure releases and collaborate with external providers and significant others to coordinate regarding issued connected to content of individualized treatment plan.
• Be amenable to supervision, including seeking consultation, as appropriate, and being invested in ongoing effort to improve service provision, and personal skill set.
• Contribute to a positive and supportive team climate.
• Supervision for LCSW and LPC licensure provided.
