Job Responsibilities:
  • Follow all department and company guidelines, stay updated on best practices, and perform duties to professional standards.
  • Help coordinate care for patients needing follow-up support after hospitalization or entering long-term care.
  • Maintain organization and meet deadlines.
  • Take leadership roles and collaborate closely with supervisors and team members.
  • Recognize areas for personal growth, seek learning opportunities, and share knowledge with colleagues.
  • Act as a resource for clinical support and support quality improvement efforts.
  • Promote safety and respect cultural differences.
  • Provide age-appropriate care addressing physical, emotional, and educational needs.
  • Maintain courtesy and professionalism with everyone, including patients, visitors, and coworkers.
  • Stay informed about relevant laws, regulations, and policies.
  • Complete all required training and support compliance efforts.
What You'll Need:
  • A master's degree in counseling, psychology, social work, family therapy, or related field.
  • One of the following Missouri licenses (or working toward it): LCSW, LPC, LMFT, PLPC, LMSW, or PLMFT.
  • If not licensed yet, agree to initiate the licensing process upon hire and follow all supervised steps.
  • Knowledge of mental health terminology, diagnosis, and treatment approaches.
  • Basic research skills and understanding of mental health theory and practice.
  • Strong communication skills and teamwork ability.
  • Good judgment, adaptability, and leadership qualities.
  • Completion of onboarding training as assigned.
  • If driving is required, meet company driving policy standards and maintain them.
