Job summary

CHE Behavioral Health Services seeks licensed mental health professionals to provide in-person psychotherapy and behavioral health services within Skilled Nursing Facilities and Assisted Living communities. This role offers flexibility with full-time, part-time, or flexible scheduling and the autonomy of private practice without business management duties.

Providers will join an established multidisciplinary team, focusing on meaningful geriatric care, with referrals and consistent patient

Qualifications

• Master's or doctoral degree in an applicable discipline required.
• Active PA license: LPC, LCSW, LMHC or LMFT with no restrictions.
• Experience with geriatric populations preferred.
• In-person psychotherapy within SNFs/assisted living settings.
• Strong communication and documentation skills required.

Responsibilities

• Provide in-person psychotherapy and behavioral health services within Skilled Nursing Facilities and Assisted Living communities.
• Conduct diagnostic assessments and develop treatment plans.
• Treat geriatric-related depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary care teams to improve patient outcomes.
• Build therapeutic relationships with residents and their families.

Education

Qualifications

Required

• Active and unrestricted Pennsylvania license as one of the following:

• LPC

• LCSW

• LMHC

• LMFT

• Master's degree or doctoral degree in an applicable discipline.

• Ability to provide compassionate, patient-centered care.

• Strong communication and documentation skills.

• Comfortable providing services in nursing home and long‑term care settings.

Preferred

• Experience working with geriatric populations.

• Experience in Skilled Nursing Facilities, rehabilitation centers, long‑term care, or assisted living settings.

• Interest in interdisciplinary and integrated healthcare.
