Job summary

Horizon Health Services in Buffalo, NY seeks a Licensed Mental Health Professional to conduct clinical intake screenings, provide referrals to behavioral health and substance use services, and follow up with clients in a supportive, patient-centered environment.

Candidates must hold NYS licensure (LMSW, LCSW, or LMHC) and have at least one year of relevant experience; strong knowledge of local systems is preferred.

Qualifications

• Must be licensed and currently registered by New York State in LMSW, LCSW, or LMHC.
• Minimum of one year of professional work experience or internship in mental health, criminal justice, or related human services.
• Strong knowledge of local behavioral health and criminal justice systems preferred.

Responsibilities

• Attend trainings via a learning management system to gain necessary knowledge.
• Conduct clinical intake screenings to assess behavioral health needs and referrals.
• Provide clinical recommendations and connect individuals to services (SPOA, FACT, SOS).
• Follow up with patients and providers to ensure engagement and address barriers to treatment.
• Maintain comprehensive resources and accurate documentation in OMH Navigator system.

As a Licensed Mental Health Professional at Horizon, you will…

• Attend trainings via our new state-of-the-art learning management system where you will gain the necessary knowledge to be a successful counselor
• Conduct clinical intake screenings to assess behavioral health needs and determine appropriate referrals at various courts in the 8th district.
• Provide clinical recommendations and connect individuals to mental health and substance use treatment services, including SPOA, FACT, and SOS teams.
• Follow up with patients and providers to ensure engagement and address barriers to treatment.
• Maintain a comprehensive guide of local behavioral health and social service resources.
• Facilitate communication between providers, courts, and state agencies.
• Educate participants on court processes and terminology.
• Assist with access to basic needs such as shelter, food, and social services benefits.
• Maintain accurate documentation and enter data into the OMH Diversion Center Navigator system.
• Coordinate with OMH to address system challenges and develop innovative solutions.

• Licensure: Must be licensed and currently registered by New York State as one of the following:
• Licensed Master Social Worker (LMSW)
• Licensed Clinical Social Worker (LCSW)
• Licensed Mental Health Counselor (LMHC)
• Experience: Minimum of one year of professional work experience or internship experience in mental health, criminal justice, substance use, or related human services required.
• Strong knowledge of local behavioral health and criminal justice systems preferred.
• Background in case management and care coordination is a plus.

Hours:

Full-time, Monday - Friday from 9:00am - 5:00pm. Schedule may vary depending on court appearances.

Location:

Regular travel between various court locations and Horizon facilities, with a designated home base at 77 Broadway St, Buffalo NY 14203.

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $52,883-$65,000. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

*This position entails eligibility for a sign-on bonus of up to $2,500 for your commitment to work within our Navigator Grant for at least one year. A change in scheduled hours or location of employment may impact eligibility of bonus.

Physical demands associated with this position include:

• Sitting for extended periods, alternating between standing and sitting
• Mobility required with repetitive wrist, hand, and finger movements
• Handling light duties, occasionally lifting objects up to 20 pounds
