MTC Care is seeking a licensed social worker to join our patient-first team serving adults and seniors in skilled nursing and assisted living facilities. You will provide individual and group psychotherapy, create personalized treatment plans, and collaborate with interdisciplinary teams to deliver high-quality behavioral health care.

Qualifikationen

Master's Degree in Social Work required.
Licensed Mental Health Counselor with the ability to practice and bill independently.
Active license to practice in State(s) where MTC Care operates.
Significant experience treating adults and/or seniors with severe persistent mental illness.
Strong communication skills and cultural humility.

Aufgaben

Provide individual and group psychotherapy sessions.
Complete mood and symptom-based screenings.
Develop individualized treatment plans for adults and/or seniors.
Provide in-service education.
Collaborate with facility staff and consult with interdisciplinary teams.
