Job summary

Amedisys is seeking a Licensed Medical Social Worker to join our home health team in Virginia. You will deliver patient- and family-centered social work services in the home, support coping strategies, and collaborate with the care team to update plans of care and coordinate transfers and discharges.

The role emphasizes timely communication of clinical changes, education on interventions, and risk assessment to ensure high-quality, patient-focused care.

Qualifications

• Holds a masters or doctorate degree in social work (CSWE-accredited).
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.

Responsibilities

• Assists patient and family with health-related financial, social and emotional concerns and helps with coping skills.
• Provides social work services per the plan of care.
• Communicates changes in clinical status to the care team and office staff in a timely manner.
• Educates patient and family on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the home and coordinates interventions with physicians and team.

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.


• Provides social work services in accordance with the plan of care.


• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.


• Educates the patient and family members and other team members on medical social work interventions and treatment plan.


• Performs care planning, transfer and discharge functions.


• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.

Qualifications
