Northshore Home Health Care in Maine seeks a Licensed Social Worker to provide compassionate psychosocial support to patients and families across hospital, school, home and community settings.

You will perform psychosocial assessments, develop individualized discharge plans, coordinate services, and collaborate with interdisciplinary teams to improve patient-centered outcomes, while ensuring meticulous documentation and compliance.
