Vanderbilt Health is seeking a social worker in Case Management to provide psychosocial assessments and targeted interventions impacting patients’ engagement with care. You will collaborate with interdisciplinary teams to meet care management goals and contribute to organizational process improvements.

Leadership for committees and guiding the care plan with population-specific knowledge are also part of the role.

Competenze

• Master's degree in social work (MSW) and licensure as required by the state.
• Experience with psychosocial assessment and brief targeted interventions preferred.
• Knowledge of patient advocacy and care coordination.
