Job summary

Human Touch Home HealthCare/ Capitol View in Washington, DC seeks a Medical Social Worker (MSW) to help patients navigate social and environmental challenges in home health settings. You will assess needs, develop care plans, and provide counseling to patients and families, coordinating with the care team.

Qualifications

• Licensed Social Worker in Washington, D.C. (LMSW or LCSW) required or preferred.
• Minimum of 1 year of clinical or medical social work experience.
• Home health or healthcare experience a plus.
• Strong communication, empathy and problem-solving skills.

Responsibilities

• Assess patients' social, emotional, and environmental needs.
• Develop individualized care plans and resource strategies.
