Social Worker (MSW) – Home Health

Location: Farmington Hills, MI 48331

What You Will Do

• Assess patients to identify psychosocial, emotional, financial, environmental, and community resource needs
• Develop individualized care plans based on patient assessments
• Provide patient and family counseling, support, and crisis intervention
• Assist with community resource coordination and referrals
• Support patients and families with advance directives and long‑term care planning resources
• Collaborate with physicians, nurses, therapists, and interdisciplinary team members to ensure coordinated care
• Participate in case conferences and care planning discussions
• Complete timely, accurate documentation in compliance with agency, state, federal, CHAP, and Joint Commission standards
