Job summary

Affinity Home Health Care Inc in Marlton, NJ seeks a Licensed Marriage and Family Therapist (LMFT) to conduct individual and group psychotherapy in skilled nursing or subacute facilities. The role requires diagnosing emotional and behavioral health conditions and developing evidence-based treatment plans in collaboration with facility staff and families.

The LMFT will maintain accurate clinical documentation in EHR systems, monitor patient progress, and adjust treatments accordingly.

Qualifications

• Active, unrestricted LMFT license in the relevant state.
• Fluent in English communication.
• Proficiency with electronic health records (EHR) and related technology.
• Strong organizational and documentation skills ensuring regulatory compliance.
• Ability to provide in-person services within facilities, including moving between rooms.
• Vaccination may be required in some facilities.

• Additional experience in psychology or addictions is a plus.
