Job summary

Carle Health seeks a Social Worker to deliver patient-centered, community-based care within an interdisciplinary healthcare team in Champaign, IL.

You will complete psychosocial assessments, develop individualized care plans, coordinate resources to address social determinants of health, provide crisis intervention, brief counseling, and advocate for patients’ rights. Documentation in EMR and participation in quality initiatives.

Qualifications

• Active LSW or equivalent state licensure required.
• Experience in healthcare, hospital, or community-based clinical settings preferred.
• Knowledge of state and federal healthcare regulations and patient rights.

Responsibilities

• Provide psychosocial assessments and develop individualized care plans for patients and families.
• Coordinate community resources, referrals, and support services to address social determinants of health.
• Collaborate with physicians, nurses, and interdisciplinary teams to support safe care transitions.
• Advocate for patients’ rights, access to services, and continuity of care across settings.
• Document assessments, interventions, and outcomes in the electronic medical record.
• Provide crisis intervention, counseling, and brief therapy within scope of licensure.
• Educate patients and families about treatment options, community supports, and coping strategies.
• Participate in quality improvement initiatives and contribute to program development.
