Qualifications

LCDCII, LCDCIII, LPC, LSW licensure required.
Responsibilities
Prepares and maintains case files for each client.
Complies with documentation requirements.
Determines appropriateness for services using DSM5/ICD 10 criteria.
Orients clients to facility/programs.
Develops Individualized Treatment Plans.
Assists clients in reviewing treatment plans.
Identifies problems and explores solutions.
Adheres to the Agency's treatment philosophy.
