Qualifications

• Associates degree required; Bachelor’s preferred.
• LCDCII, LCDCIII, LPC, LSW licensure required.

Responsibilities

• Prepares and maintains case files for each client.
• Complies with documentation requirements.
• Determines appropriateness for services using DSM5/ICD 10 criteria.
• Orients clients to facility/programs.
• Develops Individualized Treatment Plans.
• Assists clients in reviewing treatment plans.
• Identifies problems and explores solutions.
• Adheres to the Agency's treatment philosophy.
