• Ability to gather data, document problems, and establish treatment plans.
• Maintain patient confidentiality and comply with DHHS regulations.

Responsibilities

• Provide individual and group counseling to patients.
• Direct training groups and lead therapy sessions.
• Coordinate with community agencies and law enforcement for referrals and compliance.
• Complete assessments and develop individualized treatment plans with patient input.
• Maintain up-to-date patient charts and treatment goals.
• Collect urine samples for monthly urinalysis and document results.
• Support CQI monitoring and participate in case reviews.
