Job summary

EmpowerMe Wellness in Minnesota is seeking licensed therapists (PT, OT, SLP) to provide therapy and wellness services to older adults within our EmpowerMe 360 program. You’ll conduct assessments, create care plans, and deliver evidence-based interventions as part of a collaborative team.

The role focuses on helping residents maximize independence and safety with a predictable schedule, opportunities for leadership, specialty programs, and ongoing professional development.

Qualifications

• Licensure in PT, PTA, OT, COTA or SLP.
• Empowers seniors to live independently and safely.
• Strong clinical skills with a collaborative approach.
• Comfortable engaging with residents and the community.

Responsibilities

• Provide therapy and wellness services for older adults.
• Develop personalized care plans and deliver evidence-based interventions as part of a team.
• Support care coordination with wellness check-ins and caregiver communication.
• Maintain professional standards in line with company initiatives.
• Contribute to program development and quality improvement.
