Job summary

Live Well Homecare Agency, LLC is seeking a Licensed Clinical Social Worker to provide remote, insurance-based psychotherapy to adults via secure telehealth as an independent clinician. You will conduct biopsychosocial assessments, diagnose using DSM-5-TR, create and implement person-centered treatment plans, and maintain thorough EHR records while coordinating with other healthcare providers.

This role requires strong clinical judgment, telehealth proficiency, and commitment to safety and risk

Qualifications

• Must hold an active LCSW license.
• Experience providing psychotherapy to adults.
• Strong skills in assessment, DSM diagnoses, and treatment planning.
• Proficient in telehealth delivery and documenting in an EHR.
• Sound clinical judgment including safety and risk evaluation.
