Vitae Health is seeking licensed psychotherapists to provide mental health services to residents in skilled nursing facilities. The role supports overall well-being and adjustment to medical life changes, with both full-time and part-time options across the Midwest and East Coast.

As part of an interdisciplinary team, you will assess, diagnose, and treat emotional conditions, maintain HIPAA-compliant documentation, and collaborate with physicians, nursing, and social work to optimize resident

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2+ years of clinical experience preferred in geriatric, long-term care, or medical settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of SNF mental health conditions and evidence-based approaches.

Responsibilities

• Conduct comprehensive psychosocial assessments upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness.
• Support coping with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with physicians, nursing, social work, and rehab teams to coordinate care.
• Participate in care plan meetings and interdisciplinary discussions.
• Maintain HIPAA-compliant, timely clinical documentation.
• Provide crisis intervention and behavioral management as needed.
• Educate staff and families on mental health and effective communication.

Compensation:

The compensation range for this position is $35,000-$55,000, with the opportunity to earn additional income through a variable compensation (commission-based) structure, offering higher earning potential based on performance.

Our providers deliver integrated clinical services including internal medicine, psychiatry, psychotherapy, and podiatry, helping facilities meet the complex medical and behavioral health needs of their residents.

Position Summary:

Vitae Health is seeking both full-time and part-time Psychotherapists to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' overall well-being and adjustment to medical and life changes.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed

Qualifications:

• Master's degree in Psychology, Counseling, Social Work, or related field


• Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)


• 2+ years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings


• Strong clinical assessment, diagnostic, and treatment planning skills


• Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)


• Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)


• Ability to work collaboratively within an interdisciplinary team


• Excellent communication and interpersonal skills with residents, families, and staff


• Strong organizational, time management, and documentation skills


• Familiarity with electronic health records (EHR) systems preferred
