Collage Home and Community Rehabilitation Partners is seeking licensed clinical social workers to provide patient-centered neurological rehabilitation in San Jacinto, CA and surrounding areas. PRN positions available for clinicians accepting patients during their stay.

Interdisciplinary teams develop individualized plans for disabling conditions, delivered in patients’ homes and communities. Applicants must have a Master’s in Social Work and state licensure as an LCSW.

We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community.

We are looking for experienced Licensed Clinical Social Workers who want to provide truly patient-centered care in San Jacinto, CA and surrounding areas- with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
