Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in Perris, CA and surrounding areas with PRN positions available for clinicians who will accept patients for the duration of their stay.

Clinicians are supported by therapy-led leadership focused on Building Futures for patients and staff, delivering intensive, individualized home and community-based services across the continuum of brain injury to job

Qualifications

• Master's degree in Social Work required and state licensure as an LCSW.
• Excellent planning, organization, writing, and time management skills required.
• Ability to communicate with families, professionals, and co-workers is essential.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification.
