Job summary

Collage Home and Community Rehabilitation Partners in El Cajon, CA is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in patients' homes and surrounding communities. This PRN role supports ongoing treatment and coordinates with interdisciplinary teams to build tailored plans.

We treat neurological injuries and complex ortho conditions, from traumatic brain injuries to spinal injuries, focusing on functional goals in the client’s own environment.

Qualifications

• Master's degree in Social Work is required.

We are looking for experienced Licensed Clinical Social Workers who want to provide truly patient-centered care in El Cajon, CA and surrounding areas- with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
