Collage Home and Community Rehabilitation Partners in Dallas is seeking experienced Clinical Social Workers to deliver truly patient-centered care in patients' homes and communities, with PRN positions available to cover the duration of a patient’s stay.

Our interdisciplinary teams develop integrated treatment plans and address functional goals through evidence-based practices in home-based settings. A Master's in Social Work and LCSW licensure are required.

We are looking for experienced Clinical Social Workers who want to provide truly patient-centered care in Dallas, TX, and surrounding areas- with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
