Job summary

Collage Home and Community Rehabilitation Partners in Austin, TX seeks experienced Licensed Clinical Social Workers to provide patient-centered care in clients' homes and communities. PRN positions available for clinicians able to serve throughout the duration of a patient's stay.

Our interdisciplinary teams develop individualized treatment plans addressing adjustment to disability, resource linkage, and behavior modification, with an emphasis on ethics, honesty, and compassionate care.

Qualifications

• Master's in Social Work required.

We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community.

We are looking for experienced Licensed Clinical Social Workers who want to provide truly patient-centered care in Austin, TX, and surrounding areas- with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Qualifications:

• Completion of a Master's degree in Social Work and state licensure as an LCSW are required
• Excellent planning, organization, writing, and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must
