Link Rehab and Wellness seeks a Licensed Clinical Social Worker to provide clinical assessments, therapy, and counseling to geriatric residents facing mental health challenges.

You will develop individualized treatment plans, deliver evidence-based interventions, and collaborate with psychiatrists, physicians, social workers, and community partners to support comprehensive care and recovery.

Formación

• Licensed in New Jersey (LCSW) is required.
• Experience with geriatric populations preferred.
• Ability to work as part of a multidisciplinary team.

Responsabilidades

• Conduct comprehensive behavioral health assessments, including intake interviews and evaluations.
• Develop, implement, and revise individualized treatment plans based on goals and clinical needs.
• Provide evidence-based individual, group, and family therapy as appropriate.
• Coordinate with psychiatrists, physicians, social workers, and community partners to ensure coordinated care.
• Provide crisis intervention services, including de-escalation and safety planning.
• Maintain accurate and timely clinical documentation including progress notes and plans.
• Educate clients and families about mental health, coping strategies, and available resources.

Link Rehab and Wellness is looking for a Licensed Clinical Social Worker to join our team. You’ll provide clinical assessments, therapy, and counseling services to geriatric individuals experiencing mental health challenges. The clinician is responsible for developing individualized treatment plans, providing therapeutic interventions, and working as part of a multidisciplinary team to support the overall well-being of geriatric residents.

Duties and Responsibilities

• Clinical Assessment: Conduct comprehensive behavioral health assessments, including intake interviews, screenings, and evaluations to determine the mental health and/or substance use needs of clients.
• Treatment Planning: Develop, implement, and revise individualized treatment plans based on the client’s goals, preferences, and clinical needs. Ensure that plans are regularly updated and aligned with progress and setbacks.
• Therapeutic Intervention: Provide evidence-based therapeutic interventions, including individual therapy, group therapy, and family therapy as appropriate.
• Case Management: Coordinate and collaborate with other healthcare professionals, including psychiatrists, medical doctors, social workers, and community organizations, to ensure comprehensive care and resource utilization.
• Crisis Intervention: Provide crisis intervention services, including de-escalation techniques and safety planning, for clients in acute distress or experiencing mental health crises.
• Documentation and Compliance: Maintain accurate and timely clinical documentation, including progress notes and treatment plans in accordance with ethical, legal, and organizational standards.
• Client Education: Educate clients and families about mental health, coping strategies, and available resources. Encourage active participation in treatment and recovery planning.

Qualifications:

• Licensed in the state of New Jersey, such as:
• Licensed Clinical Social Worker (LCSW) required
