Job Summary

Under the Supervision of the Manager/Supervisor of Case Management, the Licensed Clinical Social Worker (LCSW) functions as an integral Team member. As member care needs are identified that require additional support outside of the member's medical needs often focused on their emotional and social needs, LCSW supports the team’s goals by interviewing members, assessing medical, psychological, emotional, and social needs to establish a course of action by exploring options and setting goals with the member, family, or caregiver. The LCSW works to ensure member care needs are augmented by community-based services and identification of additional resources by establishing a liaison role between community services and the Case management team.

Licensed Clinical Social Worker Shall
  • Counsel on Substance Abuse and rehab strategies
  • Coordinate Social Services
  • Facilitate care transitions related to behavioral health services including facility admissions, facility admission diversions, discharge to home or other living arrangements, and step down to alternate clinical care setting (i.e., residential treatment, Partial hospital, Intensive Outpatient Treatment)
  • Work closely with ACM, ICM, and Coordinators to coordinate medical, behavioral, social, and other services including but not limited to LTSS.
Required Language, License, Education, And Experience
  • Bi-Lingual English/Spanish
  • California Licensed Clinical Social Worker
  • Bachelor’s or master’s degree from an accredited school
  • Experience in multi-cultural health care delivery
Essential Duties And Responsibilities
  • Perform essential duties satisfactorily.
  • Work effectively in a fast-paced environment.
  • Displays exceptional interpersonal skills as part of a multi-disciplinary personnel team.
  • Work collaboratively with members and family/caregivers using a wellness, recovery or resilient-based approach and engage members in treatment.
  • Complete clinical assessments for mental health and other co-occurring addictive or medical disorders.
  • Monitors and coordinates the delivery of health services for members as related to behavioral care, including linking with other treatment providers.
  • Provides members with self-management skills and educational information needed so they can be full participants in their own treatment.
  • Provide and communicate recommendations for interventions.
  • Collaborate with the provider and the members.
  • Provide family/caregivers with information concerning member in person, by phone, or by correspondence as approved by Supervisor and only after receiving the member signed release of information consent for that specific individual.
  • Work collaboratively with a wide range of clinical and non-clinical disciplines to assess member and family needs, design effective care planning or service goals or medical treatment plans regarding utilization of additional resources.
  • Follow CPS case management.
  • Demonstrate multicultural skills, knowledge and experience, including the influence of cultural issues and providing services with appropriate cultural context.
  • Develops a therapeutic relationship with the members, families and caregivers, as assigned by the supervisor, to enhance effective treatment delivery.
  • Facilitate education, support groups and referrals.
  • Prepare and maintain complex treatment and progress reports and documents as needed which include diagnostic and treatment recommendations, discharge planning and follow-up services.
  • Assess members and their families in understanding and coping with emotional and social problems.
  • Advise members of available community resources and act as a liaison and/or advocate for member/families to use the available services/resources effectively.
  • Performs other duties and responsibilities as assigned or requested.
Other Proven Skills And Abilities
  • Relating in a professional manner and handle confidential information and protected health information. Performing behavioral health evaluations along with complex social and psychotherapy work with members.
  • Preparing clear concise casework records; making recommendations based on information; organizing and managing caseload.
  • Good organizational skills.
  • Proper spelling and use of medical terminology.
  • A high degree of initiative, judgement, discretion, and decision making.
