Job summary

Rehab Without Walls in San Diego and surrounding areas seeks a Licensed Clinical Social Worker (LCSW) to provide therapeutic support and coordinate care in a home-based rehabilitation environment. The role emphasizes collaboration with a rehab team to address social and emotional factors affecting patient recovery.

Ideal candidates hold CA LCSW licensure, have experience with neurological conditions (TBI/SCI/CVA), and are capable of lifting and assisting patients; a flexible field-based schedule

Qualifications

• Master's Degree in Clinical Social Work (LCSW).
• Current LCSW licensure in CA.
• Minimum 1 year rehab experience with TBI/SCI/acute diagnosis.
• CPR certification required.
• Ability to lift 50 pounds; moving/lifting up to 100 pounds following safety procedures.
• Duties require fine motor skills, visual acuity, and standing/walking for extended periods.
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and frequent positioning.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with rehab team.
• Assist physicians/health team in social/emotional aspects of care.
• Instructs/counsels patients and families on coping with illness.
• Coordinates care and identifies community resources.
• Identify and assist patients/families to use community resources to achieve objectives.

Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Instructs/counsels patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

What you'll need

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
• Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
