HealthFirst Family Care Center, Inc. is seeking a licensed clinical social worker (LCSW) to join a fast-paced medical team within a federally qualified community health center in Massachusetts.

The LCSW will provide assessment, case management, behavioral health treatment, and resource acquisition for patients referred by providers. The position requires an MA LCSW license, master’s degree in social work, CPR certification, and bilingual ability (Portuguese or Spanish preferred).

Qualifications

• Master’s Degree in Social Work from an accredited college or university.
• Massachusetts state license as a Licensed Certified Social Worker (LCSW).
• CPR certification required.
• CorI Release Form and clean background as required.

Responsibilities

• Provide assessment, consultation, case management, and behavioral health treatment.
• Coordinate resource acquisition for patients referred by providers.
• Collaborate with SBHC and HealthFirst team.

HealthFirst Family Care Center, Inc “The Heart of Our Community” provides comprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care.

LCSW Summary

Under the general supervision of the Behavioral Health Director, the Licensed Certified Social Worker (LCSW)’s function is to provide assessment, consultation, case management, behavioral health treatment and resource acquisition to patients/clients referred by the providers in conjunction with HealthFirst Family Care Center, Inc. and the School-Based Health Center (SBHC).

Schedule

Mon – Fri 8:30 am – 5:00 pm

Pay Range

$64,285.79 – $77,119.67 annually*

Required Qualifications

• Master’s Degree in Social Work from an accredited college or university.
• Position requires a Massachusetts state license as a Licensed Certified Social Worker (LCSW)
• Satisfactory completion of continuing education credits as mandated by any professional boards, licensing agencies, or credentialing organizations.
• Portuguese or Spanish fluency preferred.
• Maintain CPR certification.
• Must submit a CORI Release Form and demonstrate proof of being free from disqualifying information.
• Must have a valid driver’s license and access to an insured vehicle as some transportation may be required.
• Community health or clinical background preferred.

• Detail-oriented and accurate.
