PeopleOne Health seeks a Licensed Clinical Social Worker to join the behavioral health team supporting members across their designated area. The role works with clinicians and health coaches to address mental and behavioral health needs within a value-based primary care model.

The position emphasizes SBIRT training, community outreach, and coordination with therapy and psychiatry partners to connect members with appropriate services.

• Master’s degree in social work (MSW) from an accredited institution.
• State Licensure as a Clinical Social Worker.
• Specialized certification as Licensed Clinical Social Worker (LCSW).

• Coordinate with local substance use treatment programming for member assessments and referrals.
• Provide education for members and employees on current standards of care in mental health.

JOB SUMMARY:

PeopleOne Health has transformed healthcare by bringing together primary care, medical specialists, motivational coaching, and behavior change technology. The Licensed Clinical Social Worker position is an integral part of the behavioral health care team and will work directly with the clinical care team and health coaches to meet our members' mental and behavioral health needs. This position will serve the PeopleOne Health offices in their designated area.

Requirements

• Knowledge of EHR is a plus

EDUCATION & CERTIFICATIONS :

• Master’s degree in social work (MSW) required from an accredited institution
• State Licensure as a Clinical Social Worker
• Specialized certification as Licensed Clinical Social Worker (LCSW)

EXPERIENCE:

• 1-3 years post-Bachelor's experience preferred
• Knowledge of community resources required
• Experience working in the field of mental and behavioral health required
