Job summary

MyCHN, a nonprofit FQHC serving Greater Houston, seeks a Behavioral Health Therapist to provide comprehensive behavioral health services within an integrated care team.

You will conduct assessments, develop care plans, and offer individual, family, and crisis counseling while coordinating referrals and ensuring accurate documentation.

Texas licensure (LCSW or LPC) is required, with at least one year of clinical experience and a commitment to patient‑centered care across diverse populations.

Qualifications

• Active and unrestricted Texas license as an LCSW or LPC.
• Ability to practice independently.
• Eligibility for Medicare/Medicaid provider participation.
• At least one year of clinical experience.
• Strong communication and interpersonal skills.
• Ability to work effectively with diverse populations.
• Ability to handle sensitive situations and complex patient needs.

Responsibilities

• Conduct behavioral health assessments and develop individualized care plans.

• Collaborate with providers, psychiatrists, and interdisciplinary care teams.

• Manage follow-up care and ensure timely communication with patients.
• Support integrated care coordination across behavioral health, primary care, and community resources.
• Perform additional duties as assigned within scope.
